Before your first visit please bring the following items with you

  1. Referral letter if your were sent in by another doctor
  2. Labs or copies of any prior allergy testing if available
  3. All of your medications in their original containers or at least a
    listing of all of your medications
  4. Sinus CT or X-rays. Reports are ok.
  5. If you use a peak flow meter bring in your readings for the last
    week.
  6. All new patients must fill out a questionnaire on their first visit.
    You can speed up your time at the clinic by filling out the
    questionnaire prior to that day. To print:
